Did you know Money Follows the Person?

🏠 HOUSING OPERATORS: HERE’S ANOTHER REFERRAL PIPELINE YOU SHOULD KNOW ABOUT

Have you researched Money Follows the Person (MFP) yet?

💰 Money Follows the Person is a Medicaid demonstration designed to help eligible older adults and people with disabilities transition from institutional settings back into community-based living and services.

And here’s the important part for housing operators:

🚨 Don’t immediately ask, “How do I get MFP money?”

Instead, ask:

👉 “Who is helping these individuals transition back into the community—and who are they partnering with for housing?”

That changes the entire conversation.

MFP programs can connect with a broader ecosystem that may include:

🏥 Transition coordinators 🏠 Housing organizations ♿ Disability-service organizations 👵 Aging-service networks 🤝 Community-based providers 🏢 Public housing authorities 📋 Medicaid HCBS programs 🚐 Transportation and supportive-service organizations

CMS currently publishes a state-by-state directory of Money Follows the Person Project Directors, including direct contact information. (Medicaid.gov)

🎯 YOUR MARKETING ASSIGNMENT

This week, identify five organizations connected to this ecosystem.

Then call or email and say:

📞 “Hello, my name is [NAME]. We operate a community-based housing program in [CITY/COUNTY]. I’m researching organizations that assist eligible individuals transitioning into community living. I’d like to understand your housing referral process, the populations you serve, and what requirements you look for in community housing resources.”

Notice what we didn’t say:

❌ “Send me clients.” ❌ “How much will Medicaid pay me?” ❌ “Can I get an MFP contract?”

We started with:

✅ Relationship ✅ Education ✅ Qualification ✅ Understanding their need

That’s how professional referral pipelines are built.

🔥 REMEMBER THIS:

One referral source creates dependency. Multiple referral streams create stability.

You don’t want to just open a house.

🏠 You want to build a housing program that becomes known, trusted, and connected to the systems already serving the people who need housing.

The biggest opportunity here is the directory strategy: instead of marketing only to probation, treatment centers, and shelters, operators can start mapping the aging, disability, HCBS, independent-living, and community-transition ecosystem too.
